RESPONSIBILITIES: Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan. Allows and encourages residents to do as much of their own care as possible. May assist in preparing meals following pre-planned menus. Encourages residents to eat a proper diet and take adequate fluids while respecting the residents right to free choice. Maintains residents records daily in a timely manner and in accordance with company policy and procedures. This includes documentation related to: leisure activities; incidents and observations-charting any changes in resident’s physical condition and/or behavior; visitors; etc. Maintains a clean, safe, and orderly environment for the residents. Performs general housekeeping following cleaning schedules for resident laundry, bedrooms, dining area, living space, bathrooms, kitchen, etc. Follows proper procedures in emergency situations and responds promptly and positively to resident request for assistance including emergency pull cords, telephone calls, and request from family and friends. Encourages teamwork through cooperative interactions with co-workers during shift change. QUALIFICATIONS: Must have a valid license through the state of Florida. Must have a valid CPR certification through the American Red Cross or American Heart Association. Must be able to complete and pass a Level 2 Background Screening. Have a sympathetic attitude toward the care of the sick and elderly. Ability to carry out directions, read and write. Maturity, reliability and ability to deal effectively with the demands of the job. All applicants are required to complete a criminal background screen. JOB SUMMARY   : As a Registered Nurse with A Care Connection Home Health, you will: Provide skilled nursing care in the patient’s home for treatment of current illnesses, rehabilitative needs and preventative care Utilize a holistic approach in the provision of problem specific therapeutic interventions and teaching/training activities in accordance with the plan of care Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolutions and achieve individualized patient goals and outcomes. Collaborate with Clinical Managers and the Director of Nursing regarding issues or changes in the plan of care Accept verbal orders from physician offices and communicate these orders/directives to the clinical support team Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care Communicate effectively with patients, physicians, clinical managers and other members of the healthcare team Adhere to the provision/availability of supplies, materials and equipment needed to implement the plan of care efficiently. A Care Connection is searching for qualified and compassionate Registered Nurses/Licensed Practical Nurses for a variety of opportunities with our clients in Jacksonville and surrounding areas. This includes documentation related to: ADLs, incidents, observations and/or any changes in health status or behavior Create a clean and safe environment for each patient and follow housekeeping schedules accordingly Follow proper procedures in emergency situations and respond promptly to assistance requests including: emergency pull cords, telephone calls and requests from family and friends Cooperate with coworkers during shift change to ensure a seamless transition for the patient All LPN/RN applicants are required to complete a criminal background screening.
